Post-Surgery Home Care

Support through the fragile first weeks after a hospital discharge, when recoveries are won or lost at home.

Post-Surgery Home Care

The first weeks home decide the recovery

Hospitals fix the problem; homes are where healing happens, and where it goes wrong. Falls, missed medications, poor meals and skipped follow-ups are why so many seniors end up readmitted. A caregiver in the home closes those gaps from day one.

  • Safe mobility from day one

A steady arm for the exact moments surgeons warn about: getting out of bed, the first shower, the stairs. Support that follows the discharge instructions, not guesswork.

  • New medications on track

Discharge prescriptions collected before they are needed, schedules set up from the paperwork, and every dose prompted on time through the adjustment weeks.

  • Warning signs caught early

A caregiver who knows what the nurses said to watch for, swelling, fever, confusion, pain that trends the wrong way, and tells you the same day, not at the follow-up.

What post-surgery care includes

We are the non-medical layer of recovery, working alongside any home-health nurses or therapists your discharge plan includes.

The home, made recovery-ready

Before or on discharge day we set the house up for healing: a clear path to the bathroom, the bedroom arranged for safe transfers, groceries in the fridge and the first meals planned. Small preparation, large difference.

Daily living, handled

Bathing and dressing that respect surgical restrictions, nutritious meals that support healing, light housekeeping so the home does not deteriorate, and rides to follow-up appointments and therapy so nothing gets skipped.

A bridge to your care team

We follow the discharge instructions, keep a simple record of how each day goes, and coordinate with family and home-health clinicians. When something needs medical attention, you and they hear about it promptly.

How we help families

Three recoveries we support most often.

Joint replacements come with strict movement rules and a real fall risk in the first weeks. We keep the recovery moving and the restrictions respected.

  • Transfers and walking support that follow the surgeon’s protocol.
  • Rides to physical therapy and follow-ups.
  • Bathing help that works around the incision and restrictions.
  • Encouragement on the exercises between therapy visits.

After heart procedures and other major operations, energy is scarce and the watch-list is long. We carry the daily load so healing gets the energy.

  • Meals aligned with discharge dietary guidance.
  • Medication reminders through complex new regimens.
  • Activity paced to the recovery plan, not to habit.
  • Changes in breathing, swelling or energy reported same day.

Most recoveries fall on a working son or daughter. We cover the hours you cannot, and hand back a clear picture of the day.

  • Coverage shaped around your work schedule.
  • A simple report after every visit.
  • Overnight and weekend options for the early weeks.
  • Short-term by design: help until it is no longer needed.

Starting care

Getting help in place is simpler than most families expect. Four steps, usually completed within a day or two, and nothing to sign until you are sure.

  1. Step 1

    Conversation

Call us and tell us what is happening. No script, no pressure. We will tell you honestly whether home care is the right answer for your family.

  2. Step 2

    Assessment

A care coordinator visits your home, looks at the practical realities of the house, and talks with your loved one directly about what they want.

  3. Step 3

    Care Plan

You get a written plan with specific hours, tasks and goals, so everyone knows what success looks like. It changes as needs change.

  4. Step 4

    Your Caregiver

We match on skills and on personality, and we introduce you before care begins. If the fit is wrong, we change it at no cost.

Frequently asked questions

If you have questions about how this service works, we are here to help. These are the ones families ask us most, and you can always call for a straight answer.

Ask us anything

Often the same day. If you call while the hospital is planning the discharge, we can have the home prepared and a caregiver there when your loved one walks in the door.

No, and the two work best together. Home health sends clinicians for short skilled visits: wound checks, therapy, vitals. We provide the daily non-medical support around them: mobility, meals, medication reminders, bathing and transport. Most readmissions happen in the hours no clinician is present, and that is exactly the gap we fill.

No. Those are clinical tasks for nurses. Our caregivers support everything around them, keep an eye on the things the discharge nurses said to watch, and make sure problems get clinical attention fast.

Typically two to six weeks, tapering as strength returns. There is no contract, so you scale down or stop the moment it is no longer needed, or fold it into ongoing care if the recovery reveals a longer-term need.

Yes. The first nights home are often the hardest, and overnight or 24-hour coverage for the early stretch is one of the most common ways families use this service.

It depends on hours and duration. Because it is usually short-term, families are often surprised how manageable it is next to the cost of a readmission. The assessment is free: (562) 448-3854.
